Holiday & Absence Form

You can NO LONGER USE this form to request holidays THEY MUST BE SUBMITTED VIA YOUR ONETOUCH APP. This form is to be completed when prompted by HR to complete after 3 absences.  

Updated Absence Management Process

Effective immediately:

  • You do not need to complete an absence record form for the first or second absence in a calendar month.
  • You must complete a form if:
    1. You have 3 absences in one calendar month, OR
    2. You have 3 or more absences over a 2-month period.

Escalation Process for High Absence

  1. Monthly Review: At the start of each month, management will review absences. If you exceed the threshold, you will receive an email confirming absences and requiring:
    • Assurances on how attendance will improve
    • *Please note not responding to this email will be viewed as your response and understanding and acceptance of the notification.
  2. If absence continues into the next month:
    • Your hours may be reduced
    • You may only be up to pick up shifts without an allocated rota
    • You may not be issued a rota (no fixed timescale)

These steps will apply if assurances to improve attendance are not met and continue to exceed threshold.

Select the area or team you work in - if you work in more than one team, then select the area you complete your most hours in.
Do you confirm you are fit to return to work at normal duties?(Required)
If you select no your absence declaration cannot be accepted and you will need to speak to HR.
