Before you start
This form captures the key points of the Lydian Care Service Level Agreement in a mobile-friendly format for the service user or their authorised representative.
Please read each section carefully, complete the requested details, and submit only the signing path that applies to you.
- If the service user can sign personally, choose Yes in the capacity question later in the form.
- If someone is signing on the service user’s behalf, choose No and complete all representative details.
This summary supports the form flow but does not replace the full SLA terms already provided by Lydian Care.

The service user or representative must provide accurate information, disclose relevant changes, and help maintain a safe environment for staff to deliver care.
- Staff must be treated respectfully and not exposed to abusive behaviour, unsafe conditions, smoking, vaping, illegal substances, or unmanaged pets during visits.
- Specific visit times cannot be guaranteed, and welfare procedures may be followed if access cannot be gained or safety concerns arise.
Privacy, complaints, fees and review
Personal information may be collected, stored, used, retained, and shared for the assessment, planning, delivery, review, and management of care in line with UK GDPR, the Data Protection Act 2018, safeguarding duties, and other legal obligations.
Concerns or complaints can be raised with the Lydian Care office, through the formal complaints procedure, with the social worker, or with the RQIA.
- Services may be reviewed periodically and at least annually.
- Private-service fees and cancellation terms may apply where relevant and are communicated separately.
- Either party may end the agreement with reasonable notice, although Lydian Care may act immediately where serious safety concerns arise.
Surveillance and consent guidance
Please tell Lydian Care about any CCTV, cameras, doorbell cameras, audio recording devices, or other monitoring equipment in or around the property.
Choose one surveillance option below. If surveillance devices are in use, you must provide details.
- If the service user cannot sign personally, the representative details later in the form become required.
- The declaration confirms that the signer has read, understood, and agrees to the SLA terms and has received the Service User Guide.
